SPRING LAKE ACCIDENT POLICY AND PROCEDURES.

• You must report your injury to your supervisor or our EMT’s within 24 hours.
• You must fill out the FIRST REPORT OF INJURY form in this time period.
• If you need treatment you must go through the Municipality’s doctors unless it is an emergency and in which case you go to the hospital emergency room. 
• You must hand in all paperwork from the doctors and report any guidelines given from the doctor to you as far as your ability to work.
